Does the declared-value cap survive if the forum-selection clause fails?

They are severable. The Ninth Circuit treats the forum-selection clause as unenforceable only where it offends a strong public policy of the forum, and that holding does not reach the liability ceiling, which is fixed by statute rather than by the parties’ choice of venue. On your facts the cap therefore stands even if venue moves.

One caveat worth carrying into the memo: your sources disagree. The treatise records an unreconciled split between the Second and Ninth Circuits on declared-value ceilings in intermodal carriage, which is not addressed in either of the primary authorities above.
